Procedure for Applying for State Certification Exam

The next step after course completion is to appear and pass state certification exam within two-year eligibility period. This exam includes two sections, namely a written test section and clinical a CNA skills evaluation test section.

The former section is composed of 70 multiple choice questions, which one must complete within 2 hours. Instead of taking written test, provision for taking oral test is also available.

For taking oral test, you will require to request (highlight) it in the exam application form. Oral test includes solving 60 multiple choice questions (first part) and solving 10 multiple choice questions associated with recognizing commonly used English nursing expressions (words).

Skills test intends to test your clinical hands-on skills ability. Requiring students to perform any 5 skills within 30 minutes, skills are arbitrarily selected and assigned by a proctor, who also supervises and rates their performance in each skill.

Out of the five skills assigned, the measurement skill will be asked for sure. Give your best in (performing) each skill, as failure in even a single one will lead you towards retaking skills test next time.

Passing in both the parts of the exam (written test and skills test) will bestow you with a CNA license to work in any sector of health care industry in the State.

The Nurse Aide Registry provides this license. You can apply for this exam thrice during 2-year validity period.

Failing in written test or skills test in first or second attempt will require you retest that test only. You will require retaking course and certification exam if you fail in the exam in your third try.

Irrespective of the state you are working in as a Certified Nurse Aide, you are required to renew your license after every 2 years. Doing this will allow you to continue your employment in that state.

To renew a license in Wisconsin, you will require proving that you have worked for at least eight hours as a certified nursing assistant in any authorized facility over the last 2 years in the State.

Wisconsin CNA Reciprocity

To transfer to Wisconsin in order to work as a certified nurse aide, you will need to attain certification by submitting filled reciprocity form.

This form is available on state’s Department of Health Services site. This form consists of two parts.

You require filling all the sections of the first part and submitting it to your home state nurse aide registry for the second part completion, after which it is sent to Wisconsin Department of Health Services for processing.

Following things must be proved for transfer approval:

  • Valid certificate from home state
  • Good reputation in the state
  • Must have completed a state-approved nurse assistant training program, consisting of 120 hours of training (including 32 hours of clinical training)
  • Clean criminal record

These documents will be verified and you will be informed about the status of your Transfer CNA License. You will acquire a license if your transfer is approved after verification.
