Procedure for applying for State certification exam

You will be required to pass the state certification exam to get a license in following the successful completion of your nurse aide course. This exam includes two parts: written part and clinical skills evaluation part. This test can also be taken in viva, but must be specified in the application form prior to exam appearance. The performance of the candidate will be tested in any 5 arbitrarily selected skills from a list of 25 skills, invigilated by the administrator during the skills evaluation test. The Indiana State Department of Health (ISDH) – Division of Long-Term Care will require the candidate to renew his/her license once after a gap of every two years. To apply for renewal, you will be required to prove that you have worked as a certified nursing assistant for eight hours over the last 2 years and have 12-hour on-the-job training experience per year over that period. This condition must be fulfilled to guarantee your license renewal.

Transfer of an out-of-state candidate to Indiana can be done by contacting ISDH. This will require the candidate to follow and meet the following procedure:

  • Sending Nurse Aide Competency Evaluation Application for approval
